HD Physical Therapy shares your concern about the protection of online privacy for children as web site visitors as well as your children’s access to inappropriate content. We do not knowingly or purposefully collect personally identifiable information about children under 13. To request the deletion of your child’s contact information from our database, please call HD Physical Therapy at 781.587.0776.

3.) Your e-mail address
4.) The phrase “Updated Contact Information”

To Remove Your Information from Our Files

If you do not wish to receive information from HD Physical Therapy, please let us know by sending an e-mail notification to info@HDPTonline.com. When you make a request for removal, you will need to provide the following information:
1.) Your full name.
2.) Your e-mail address.
3.) A statement asking that HD Physical Therapy remove your e-mail address from our files.
If you use more than one e-mail address, send a message to us from each of your e-mail accounts.
